You can contact your GP for copies of your childhood Immunisation records or documents. If you have worked in the same or similar role you are applying for now, your previous employer’s Occupational Health department should also have a copy of your immunisation documents or records. Please call or email either your GP or your previous employer to find out how to request this information from them.
If you can't get your immunisation records, you'll need to let Optima Health (our Occupational Health provider) know so they can arrange a clinic appointment for you.
