If you are missing a job role on your profile, you may not be able to see shifts. If you believe this might be the case, you will need to discuss this with an appropriate manager. If they agree, they will need to complete the relevant form below.
This manager must have the authority to approve you to work under the requested code. For example, if you request an A&E code, a clinical manager within an A&E setting must approve it.

If you are a Substantive or Referral Bank Member, you manager must send the form to your local Client Services team.
If you are a Bank recruited worker, please email the Job Role Amendment for Bank recruited workers form with your updated CV to your local Client Services team. If you do not supply an updated CV or your form is not fully completed, your request will likely be denied.
