How do I notify you I am pregnant?

To let us know about your pregnancy, please send us one of the below: 

  • A copy of both sides of your MAT B1 form (this is your maternity certificate which will be issued to you after 20 weeks of pregnancy by your doctor or midwife)  
  • Letter from the doctor or midwife confirming your pregnancy
  • Babies birth certificate (in the event of early arrival)

 

I'm working on a Direct Engagement agreement

If you are working on a Direct Engagement agreement, please complete our Maternity Notification Form for Direct Engagement Workers and send it with your documents to your relevant recruitment team.

 

I am NOT on a Direct Engagement agreement

If you are not working on a Direct Engagement agreement, please send these to payandreward@nhsprofessionals.nhs.uk.

You will also need to complete a Maternity leave notification form by the end of the 15th week before the expected week your baby is due (or as soon as you can).
