This form is also known as a Declaration of Health.

Complete the form
Please answer all questions to the best of your ability
You have 30 days to complete the form
We recommend completing it within 2 weeks of receiving the email to avoid any delay to your application.
If you don't complete the form within 30 days your application will be withdrawn.
Optima Health assess your questionnaire
Optima Health will assess your questionnaire as quickly as possible
They will also advise you of any next steps.
If you are a Bank Exclusive applicant, as long as you have completed all your other recruitment tasks and your Declaration of Health, we will review any milestone dates related to Occupational Health so you can continue to book shifts.
Submitting Evidence
You only need to submit evidence of your immunisations if you are requested to do so by Optima Health.
If you need to send evidence of immunisations, Optima Health will tell you how to send your documents when arranging a clinic with you.
