- You'll receive an email from Optima Health - our Occupational Health provider
- The email includes:
- Your login details, and
- A link to your pre-placement questionnaire.
- This form is also known as a Declaration of Health.
- Please make sure you regularly check your email inbox and junk mail folders to ensure you don't miss this email.
Complete the form
- Please answer all questions to the best of your ability
- We recommend completing it within 2 weeks of receiving the email to avoid any delay to your application.
Optima Health assess your questionnaire
- Optima Health will assess your questionnaire as quickly as possible
- They will also advise you of any next steps.
Submitting Evidence
- You only need to submit evidence of your immunisations if you are requested to do so by Optima Health.
- If you need to send evidence of immunisations, Optima Health will tell you how to send your documents when arranging a clinic with you.
- If you can't find this information or have mislaid it, please contact Optima Health directly.
