Please download and complete an ‘External Validation Consent’ form if you do not have:
- a UK or Irish Passport
- a UK or Irish Driving Licence, or
- a UK Birth Certificate issued within one year of your birth.
You will also need to provide:
A passport size photograph that:
- Has been signed on the back by a Person of Standing
- Has the Person of Standing's full name on the back
- The date of signature
What is a Person of Standing?
All Persons of standing must:
- work in (or be retired from) a recognised profession
- have known you for at least two years in a professional or personal capacity and be able to confirm your identity.
They should not:
- be closely related to you by birth or marriage
- be involved in a relationship with you
- live at the same address as you
For an example list of people who can act as your Person of Standing, please download this list from NHS Employers.
A letter from the same person stating that they have known you for at least 2 years that:
- Must be on Company Headed paper or sent from a work email address
- States that they know you (stating your full name)
- States how many years they have known you for
- Includes the phrase "I certify the accompanying photo to be a true likeness of this individual"
- Includes their name
- Includes their place of employment
- Includes their phone number
- Is dated the same as the verified passport photo
