How should I complete my DBS form?
On your DBS form, please enter whatever your current address is.
My address has already changed since I completed the application form to join NHSP

My address has not yet changed but will do soon
Please complete your DBS form as usual as set-out in our DBS application guide. Once you have moved please let us know your new address as soon as possible using the 'Need More Help?' Contact Form below.
If we've not yet had a response from the Disclosure and Barring Service by the time you notify us of your new address, we may need to see new documents to prove you are living at your new address. If this is the case, we will email you to explain what we will need.
I have received my DBS certificate but I have moved since. How do I let you know?
If you can book shifts through My:Bank, you can change your address in My:Bank. For more information, please see our article My phone number, home address or email address has changed
If you can't book shifts through My:Bank just yet and you have received your DBS certificate, your DBS check is considered valid and will not need to be resubmitted. However, please let us know your new address as soon as possible using the 'Need More Help?' Contact Form below.
If you are on the DBS Update Service, you should also let the Disclosure and Barring Service know your new address by logging into the DBS Update Service Portal.
